Our goal is to provide the best quality dentistry and a relationship built on trust. In order to assist you with your health care investment, we are providing the following information:
Payment is expected at time of service. We accept cash, check, Visa, Mastercard, Discover, and American Express. We also accept Care Credit, For your conveniences, our office will be happy to assist you with the application process, it is also our policy that all estimated copayments and deductibles are paid at time of service. We reserve the right to commence collection or legal proceedings on accounts 90 days past due. You may be responsible for late fees and collection costs associated with the collection process.
As a courtesy, we will file insurance claims on your behalf based on the information you provide. A copy of your current insurance card and all necessary information is requested for accurate submission. It is patient’s responsibility to keep our office updated with any changes to your insurance policy. Estimates are based on basic information given to us and are subjected to change by your insurance. Any estimate you have received by our office is not a guarantee from your insurance. All of our providers will recommend treatment based on your dental health not your insurance coverage.
Should a remaining balance exist after insurance has paid, a statement will be mailed to the address on file, Payment is expected with 30 days of statement date.
Reviewing your dental insurance policy handbook is highly recommended.
Some of our services may be covered by your medical insurance. A copy of your current medical insurance card and all necessary information is requested.
If you are a new patient, we will take X-rays as part of the initial exam and to establish a baseline.
Current x-rays (within the past 12 months) will be accepted if present at time of service/exam.
Existing patients will require updating x-rays yearly to assists in monitoring for possible changes.
Our office appreciates a 48 hours notice during business hours for all cancellation appointments.
Multiple cancellations or no show are subject to a $25 fee per patient, per incident.
In Case of a Minor
The adult accompanying the minor is responsible for payment on the account. Non-emergency treatment will be denied unless charges have been pre-paid.
